Are the song titles supposed to have dashes at the beginning of every one?

Yes and no!

QCD provides title and artist from the ID3 tags.
If a file does not contain any of these informations, QCD returns the filename as title. Because <#SongTitle> is made of "<#artist> - <#title>" you get an empty <#artist>, a "-" and the filename.
So the best way for would be if you create the tags for your songs.
I prefer MP3Tag (including english LangPack). Great piece of software! It can create ID3 tags from filenames.

So it's no bug.  :-D

ps: I'll suppress the dash in <#songtitle> if <#artist> is empty in a next version.

Okay, how about this one:

<#SongTitle> works fine, but, using <#artist> or <#title> causes just the text (i.e. "artist" or "title") to be displayed in red.

No, it works! I just tried it.
Maybe there is an other error in your HTML code.

No bug...   :-p

The file works! Maybe because you did not use <#artist> or <#title> ...

 "Schließen" means "close". But I found that major bug...

When exactly does the 404 (file not found) error appear?

It 404's immediately after login (when the login window closes).
